If you prefer to register by mail or in-person at a game, please print and complete our <a href=\"https:\/\/powerhockey.com\/minnesota\/site\/wp-content\/uploads\/MPHL-2019-Registration-v1.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">2019 MPHL Registration Form<\/a>.<\/strong><\/p>\n<p><strong>For the <a href=\"https:\/\/powerhockey.com\/minnesota\/site\/wp-content\/uploads\/MPHL-2019-Roster.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">2019 MPHL Roster<\/a> please click here.<\/strong><\/p>\n<p><strong><span style=\"text-decoration: underline;\">Player Eligibility Notice: <\/span><\/strong><br \/>\nTo participate in the MPHL players must:<\/p>\n<ul>\n<li>Medically require the use of a power wheelchair in daily life<\/li>\n<li>Display cognitive ability to understand and follow basic hockey rules<\/li>\n<li>Be age 13 or older by June 8th, 2019<\/li>\n<li>Be accompanied by an adult (Family, Guardian or Caregiver) if under the age of 16<\/li>\n<li>Be independent in aspects of personal cares or be accompanied by their own caregiver \/ assistant<br \/>\n<em><strong>The MPHL does not provide caregivers \/ assistants for players<\/strong><\/em><\/li>\n<li>Complete and sign <a href=\"https:\/\/powerhockey.com\/minnesota\/site\/wp-content\/uploads\/MPHL-2019-Liability-Waiver-Media-Release.pdf\" target=\"_blank\" rel=\"noopener noreferrer\"><strong>Insurance Liability \/ Media Release Form<\/strong><\/a><\/li>\n<\/ul>\n<p><strong><span style=\"text-decoration: underline;\">Registration Fee:<\/span><\/strong><strong> $50 for entire season. \u00a0Online payments are accepted.
